Key Points
-
Adult pleural infection is associated with high morbidity and mortality Combination intra-pleural fibrinolytic therapy should be considered when:
-
standard medical treatment (intercostal drainage and antibiotics) has failed to achieve satisfactory clearance of the pleural space after 12-24 hours, and surgical intervention is considered clinically inappropriate or will be significantly delayed potentially leading to a negative impact on patient outcome.
-
Alteplase (rt-PA) and Dornase alfa (rhDNase) must be used in combination. Either used in isolation is ineffective. Written consent should be taken prior to the administration of intra-pleural fibrinolytics in all cases
-
A Respiratory Consultant must be involved in the patient’s care prior to administration, and it should only occur in an appropriate specialist environment (respiratory ward, MADU or the intensive care unit)
