Process for transitioning children and young people from paediatric to adolescent diabetes secondary care

All children and young people with Diabetes will be cared for by the Paediatric Diabetes team from diagnosis until the year of their 18th Birthday, unless the young person requests earlier transfer. This includes acute admission to the paediatric ward if necessary; the young person can request admission to an adult ward from the age of 16 years onwards.

All patients will be offered a phased introduction to the Adult Diabetes team jointly with Paediatric MDT team from age 16 years onwards and will be transitioned into adult service from age 16 to 18 years. The concept of transition and young people self-managing their own diabetes will be introduced and developed from school year 7 onwards (11 years of age).

In the year of their 16th birthday, diabetes care will be transferred to the Transition Service at King Edward VII Hospital, where the Adult Diabetes service is sited. The Paediatric and Young Adult Diabetes MDT will be present in the transition clinic. These are held once a month, so CYP have 18 months to 2 years joint adult/paediatric care in clinic before transfer to the adolescent services. However, care remains under the paediatric team, including out of hours and for hospital admissions.

Parents will be allowed to bring the young people to the transition clinics, but patients will generally be treated as adults and encouraged to come into the consultation on their own, with parents seen separately if required.

The transition process will be led by Dr S Edate, Paediatric lead for transition and Dr Baburaj, lead Adolescent Diabetes Consultant. If, during this process the patient is felt not to be ready for transition, additional appointments will be arranged and psychology support offered.

This guideline is applicable to Berkshire catchment and patients from preselected GP practices in Buckinghamshire as per stipulated agreement with Buckinghamshire CCG.

The process of transition of young people with Diabetes from rest of Buckinghamshire currently involves transfer without a joint clinic so this is under discussion with the adult Diabetes team based in Buckinghamshire.
