Go to the homepage of the Trust intranet, ourplace

Type “psych” into the Forms search box and it will find the “Psychiatric liaison referral form – FPH” form

Click the link to access the e-form

Complete all fields of the form with as much detail as you can and then “submit”

Please also bleep the team on 5255 to inform them of the referral
